The Health Information Technology Toolkit for Home Health Agencies was designed to help home health agencies, private duty nursing services, and home-based hospices, and other organizations providing health care services in the home; assess their abilities to plan, select, implement, effectively use and exchange important information about patient care. The toolkit contains numerous resources, including tools for hospice, telehealth, health information exchange, and personal health records.

The Health Information Technology (HIT) Toolkit for Critical Access and Small Hospitals

The HIT Toolkit for Critical Access and Small Hospitals toolkit is designed to provide tested tools, that will assist with planning and making the right choices when implementing an HIT program in a hospital. It consists of seven stages categorized into three main sections: Assess: Plan – Select, Utilize: Implement - Effective Use and Exchange: Readiness – Interoperate. Each tool includes a brief statement of purpose, instructions for use of the tool and tool structure. The toolkit is available as several Word files requiring Microsoft Word to view the files.

The various resources in this toolkit explain how health information technology (IT) affects health care, and it also provides analyst’s interpretations of the value of these technologies. Progress in patient safety, quality care, efficiency and data collection can be achieved through Health IT. This toolkit’s resources include “story ideas, selected experts with contact information, selected websites of interest and a glossary".

Health Information Technology Toolkit for Rural Health Care Providers

Developed by the Rural Assistance Center, the HIT toolkit is designed to provide federal resources to rural health facilities and providers. The toolkit helps health facilities choose, implement, and operate HIT systems. Rural comunities will be able to; address the issue of gaining capital funding, staying informed of EHR-related legistlation, help community colleges in advancing their training programs, and leveraging federal resources for project development.

Health Information Technology Toolbox

The Health IT Adoption Toolbox is a compilation of planning, implementation and evaluation resources to help community health centers, other safety net providers and ambulatory care providers implement health IT applications in their facilities. The toolbox is organized to support a range of healthcare providers, from senior management to the staff charged with implementing health information systems. The toolkit is available as a group of several web pages and requires a web browser to view it.

E-Louisiana HIT Toolkit
