The elements in the toolkit help to increase outcomes, such as health status, timeliness of care, family centeredness, and family functioning in a medical home. Achieving the standards in the toolkit is a requirement to be in the National Committee for Quality Assurance Physician Practice Connections® Patient Centered Medical Home Recognition program. The toolkit was developed by the American Academy of Pediatrics and requires registration. The web-based toolkit requires a web browser.
