Health Care Transition - Toolkit for Families of YSHCN

The purpose of this toolkit is to ensure that Youth with Special Health Care Needs (YSHCN) have a smooth transition into adult health care systems. This toolkit contains information to support the goal to increase the number of medical providers who have knowledge and expertise in providing quality medical services to YSHCN and who are able to support the transition from pediatric to adult health care systems within a medical home.
