Searching for Innovators: MX is on the Hunt!
We are excited to announce that MX is collaborating with the California HealthCare Foundation (CHCF) to find best practices or “better ideas” to improve the care of patients with chronic illnesses.
With the Centers for Disease Control and Prevention (CDC) reporting that seven out of every 10 deaths are related to chronic diseases each year in the U.S., health care innovators are seeking ways to solve this burgeoning crisis.
CHCF is using the MX Hospital Channel to identify and produce full Case Studies for the best models treating Complex Chronic Illnesses, and may select and package some for easy transfer and implementation.
MX is searching for complex chronic illness programs/models, currently in use, and treating two or more of the following: Diabetes Mellitus, Hypertension, Coronary Artery Disease, Heart Failure, Lung Failure – Asthma, COPD, Vascular Disease, Cancer, Stroke, Chronic Renal Disease (including renal failure), Arthritis, Dementia, Depression, Degenerative Neurological disorders and other severe mental illnesses (Bipolar, Schizophrenia.)
Help contribute to a new series sponsored by the California HealthCare Foundation of successful care models for individuals with multiple chronic conditions. Honorarium is available to participating organizations. The published case studies will be available to the public at no cost via MX.com and CHCF.
