This Toolkit provides a starting point for conversations regarding how to better serve diverse populations of older adults. Health Information Technology Toolkit for Physician Offices

Stratis Health developed the Health Information Technology Toolkit for Physicians to assist them to assess their readiness and plan, implement, make effective use of and exchange critical information about their patients. The toolkit includes various resources, including: tools for telehealth, health information exchange and personal health records. The toolkit includes web-based components, Word documents, and PowerPoint documents, requiring a web browser, Microsoft Word, and Microsoft PowerPoint.

2011 Meaningful Use Tool

Developed by the National Institutes of Health, the 2011 Meaningful Use Tool is used to help prepare and assess candidates for certification. This tool requires a web browser and a downloaded desktop application (or a Java class library).

Health Information Technology Toolkit for Rural Health Care Providers

Developed by the Rural Assistance Center, the HIT toolkit is designed to provide federal resources to rural health facilities and providers. The toolkit helps health facilities choose, implement, and operate HIT systems. Rural comunities will be able to; address the issue of gaining capital funding, staying informed of EHR-related legistlation, help community colleges in advancing their training programs, and leveraging federal resources for project development.

AACN Healthy Work Environment Assessment

Developed by the American Association of Critical-Care Nurses, the AACN Healthy Work Environment Assessment tool is a screening test for organizations to measure the health of their work environment against the AACN Standards for Establishing and Sustaining Healthy Work Environments. It provides an overall healthy work environment score and scores for each of the six essential standards. This screening tool evaluates a work environment based on each of the standards, but does not diagnose specific challenges within a standard.
