Developed by the U.S. Department of Defense Military Health System, the Situation, Background, Assessment, Recommendation (SBAR) Toolkit is a structured communication tool that can improve information exchange within a healthcare organization and reduce the rate of adverse events. The toolkit is available as several PDFs, PowerPoint files, Word files, and Windows Media Videos (WMVs), requiring Adobe Reader, Microsoft PowerPoint, Microsoft Word, and Windows Media Player, respectively.

The 5 Million Lives Campaign - Getting Started Kit: Prevent Adverse Drug Events (Medication Reconciliation) How-to Guide

A national initiative led by IHI, the 5 Million Lives Campaign works to improve the quality of the nation's health care by protecting patients from five million incidents of medical harm. This initiative focuses on preventing adverse drug events. This How-to Guide toolkit provides strategies to decrease and prevent any harm to those that are under the related services. Patient Safety Primers

Patient Safety Primers are web-based guides for health care professionals on safety ideas to put into action in hospitals and health care settings. Each tool refers to a specific topic and provides detailed background information as well as current resources and links. The toolkit was developed by the University of California, San Francisco Medical Center. A web browser is required to view this guide.

Patient Safety and Quality: An Evidence-Based Handbook for Nurses

The Agency for Healthcare Research and Quality (AHRQ) and the Robert Wood Johnson Foundation (RWJF) have jointly sponsored the development of this handbook for nurses on patient safety and quality. Patient Safety and Quality: An Evidence-Based Handbook for Nurses examines the broad range of issues involved in providing high quality and safe care across health care settings. The handbook is available as several PDFs, requiring Adobe Reader.

Readiness Assessment Tool for Nursing Homes

The Readiness Assessment Tool is designed to help nursing homes assess whether they are ready to undertake organization-wide practice and culture changes for Quality Improvement (QI). The information obtained through the baseline assessment from use of the tool can form the basis of dialogue and problem-solving across all levels of staff to enhance an organization's readiness for QI activity. It is based on the root cause analysis (RCA) approach, and it is primarily a learning exercise. The tool is available as a PDF, requiring Adobe Reader.

Common Formats for Patient Safety Event Reporting

The Common Formats were authorized by the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act). They facilitate Patient Safety Organizations’ (PSOs) collection of patient safety work product from health care providers in a standardized manner. The formats are being used by health care providers and PSOs to report a comprehensive range of patient safety concerns, capturing both structured and narrative information. The tool was developed by the Agency for Healthcare Research and Quality. The tool is available as a PDF, requiring Adobe Reader.
