The Quality Indicators Toolkit for Hospitals was developed by AHRQ. The toolkit was designed for hospitals in helping them understand Quality Indicators and how to use them in order to advance quality of care and patient safety. The focus of the toolkit is on the 17 Patient Safety Indicators and the 28 Inpatient Quality Indicators. The toolkit is available as a PDF, required Adobe Reader.

Immunization of Hospital Health Care Workers - Seasonal Influenza - Toolkit

The Kansas Healthcare Collaborative created this online toolkit to address the spread of influenza and to protect the health of both patients and health care workers. Transmission of influenza has been documented among many patient populations, and infections have been linked to unvaccinated health care workers. The Joint Commission on Accreditation of Healthcare Organizations adopted an infection control standard that requires accredited organizations to offer influenza vaccinations to staff, volunteers, and licensed independent practitioners with close patient contact.

Safe Critical Care Project: Testing Improvement Strategies

PIPS compiled a series of tools to assess the effectiveness of hospital strategies to combat blood stream infections and ventilator-associated pneumonia. This Web-based toolkit focuses on two interventions to improve critical care—reduction of catheter-related bloodstream infections and ventilator-associated pneumonia. The toolkit includes: Education, Inventories (surveys), Paper checklists, Statistical process control, Surveillance guides and Quality improvement.

Patient Safety Toolbox

The NASHP, supported by the Commonwealth Fund, developed this online toolkit to be used or modified to help organizations develop or improve adverse event reporting systems.  The toolkit includes: Background Information, Data Collection Tools, Data Analysis Tools, Data Feedback Tools, NASHP Publications on Patient Safety and a State Map. Making Strides in Safety®

The AMA is working with the Institute for Healthcare Improvement (IHI) in its nationally based “100,000 Lives Campaign,” to encourages physicians to use evidence-based interventions in their hospitals to improve the way certain medical conditions are handled. This guide for patients can help them care for themselves when they leave the hospital. The easy-to-read guide can be used by both hospital staff and patients during the discharge process and provides a way for patients to track important information. RWJF developed "Expecting Success: Excellence in Cardiac Care" to focus on poor quality and inequality in care provided in U.S. hospitals and to assist and provide guides for hospitals to grade the quality of cardiac treatment they provide to patients. This toolkit targets ways of improving quality of cardiac care while reducing racial, ethnic and language disparities.
