Studer Group Toolkit: Patient Safety

The Studer created this toolkit to provide health care organizations with devices that can be put into action to improve patient safety and these devices can be integrated into an organization’s current operations. This toolkit will guide healthcare providers to improve the way a meeting is run, how to talk with patients and how to interview prospective employees. It will also assist with better utilization of time and address national standards as set forth by organizations such as the Joint Commission and IHI. Patient Safety Program - Healthcare Communications Toolkit to Improve Transitions in Care

The Handoff Communications Toolkit was designed by the Department of Defense (DoD) Patient Safety Program and is based on the TEAMSTEPPS (Team Strategies and Tools to Enhance Performance and Patient Safety) curriculum. TEAMSTEPPS is an evidence-based curriculum designed to educate multi-disciplinary medical staff and personnel on critical team performance skills, including communication concepts to improve patient safety and quality.

Safe Injection Practices Toolkit

The ASC Quality Collaboration has created this toolkit to improve the current administration of injections. Safe injection practices are critical to patient safety and also protect healthcare providers. There is a risk that Hepatitis C and hepatitis B viruses, as well as HIV can be spread from patient to patient without improvements in the injection practices. There are two toolkits – the “BASIC safe Injection Practices” and the “EXPANDED Safe Injection Practices." The toolkit is available as a web page and Word files, and requires a web browser and Microsoft Word to view them.

A Reporter's Toolkit: Health Information Technology

This web-based toolkit was compiled and written by Sam Takvorian. The various resources in this toolkit explain how health information technology (IT) affects health care, and it also provides analyst’s interpretations of the value of these technologies. Progress in patient safety, quality care, efficiency and data collection can be achieved through Health IT. This toolkit’s resources include “story ideas, selected experts with contact information, selected websites of interest and a glossary".

MERS Event Reporting

Developed by GE Healthcare and patient safety experts, the Medical Event Reporting System (MERS) enables hospitals to manage data on medical errors, and adverse and near-miss events. The system provides a way to record and analyze this data. A video demonstration of the program is also available. A web browser is required to view the system overview and video demonstration. 20 Tips to Help Prevent Medical Errors: Patient Fact Sheet

Developed by the AHRQ, this Patient Fact Sheet informs patients on how to get safer care in order to prevent medical errors. The topics covered include; medication, hospital stays, surgery, and other steps. The Solution requires a web browser. It is also available as a PDF, requiring Adobe Reader.

Medications at Transitions and Clinical Handoffs (MATCH) Toolkit for Medication Reconciliation

The MATCH toolkit was developed by Northwestern Memorial Hospital (Chicago, IL), AHRQ, Northwestern University Feinberg School of Medicine, and the Joint Commission. This toolkit addresses the need for ensuring accurate management of patients' medication regimes in the process of a clinical handoff to avoid medication discrepancies and to ensure patient safety. Developed by the AHRQ, this toolkit provides a booklet and video on the topic of blood thinner medicines for patients. The video explains how blood thinners work in an easy-to-understand manner, and why they should be taken correctly. The booklet explains how blood thinner pills can help in preventing blood clot formation and what patients can expect from taking these medications. Both items are available in English and Spanish. Adobe Reader and a media player are required for this toolkit.
