Adolescents Health Toolkit: For Providers

The Governor's Office of Children's Care Coordination developed this online toolkit to strengthen adolescent Early Periodic Screening, Diagnosis, and Treatment (EPSDT) services and to improve the quality of Adolescent care. The following tools were created to assist providers in offering quality care to Adolescents. The toolkit includes: Components of annual visits and what they should include; recommended screenings that should be performed; guidelines for screening and counseling related to health and disease-specific conditions; tools to screen for emotional or behavioral disorders; questions developed to screen adolescents for high risk alcohol and other drug use; addressing Immunization status and how to proceed; GAPS questionnaires to gather information about the reason for the visit, health concerns and health risk behaviors; education about nutrition, education, safety, and family lifestyle choices; handouts regarding common health conditions and information about parenting Adolescents; and additional topics and resources related to Adolescent care.
