The Adverse Event Monitoring and Tracking Tool represents a collection of items to help implement IHI GTT methodology for tracking adverse events in a hospital system. This Solution is currently in-use at Adventist Health System, including the world-recognized Florida Hospital.
OBJECTIVES:
How are the components of the Adverse Event Monitoring and Tracking Tool used?
The Adverse Event Monitoring and Tracking Tool can be used to prepare, educate, train and manage the entire Adverse Event Monitoring and Tracking project. Included templates are provided as a foundation for facilities to customize the materials for their unique requirements. The manual is divided into sections, each containing the forms, tools or documents needed for the different stages of Adverse Event Monitoring and Tracking management and implementation. A complete guidebook is included in the first section of the manual to navigate the user though the process.
Records are retrieved and randomized to obtain 20 records per month for a facility. Two analyst reviewers review records to find harms, as defined by the IHI, using the Monitoring and Tracking worksheets. Once harms are identified the two analysts meet to find consensus and summarize the findings to present to a physician who authenticates the adverse event. Findings are inputted to the database. Reports are generated quarterly and reported to the facility.

The guidelines were created in-house to aid the nurse reviewers.

What does it take to implement? Maintain?
The Adverse Event Monitoring and Tracking Tool requires long-term commitment to implement and deploy. A minimum of 4 weeks in needed to train reviewers and initiate record reviews. A minimum of 12 months of data collection is recommended before reports are generated. The Solution materials provide an initial foundation. The ongoing data results need leadership champions combined with ongoing data monitoring.

Adventist Health System adopted the IHI Global Trigger Tool for Measuring Adverse Events as their primary methodology to measure harms occurring in our facilities.
