The Center for Health Care Strategies (CHCS) has worked extensively with leading states and health plans, as well as federal policymakers to lead new advances for integrating Medicare and Medicaid services. This online toolkit contains resources for integrating care and includes policy-related materials, hands-on tools to help guide state efforts, as well as sample documents from states that have successfully implemented these new advances. Medicaid Return on Investment Template

The University of North Carolina at Chapel Hill developed the Return on Investment (ROI) Template. This Microsoft Excel-based tool is designed for use by states and health plans to measure ROI of quality improvement efforts. The tool includes an Instruction Manual, the Medicaid ROI Template, and an Example Template. The Instruction Manual is available as a PDF, requiring Adobe Reader to view the document. Transition Coaches Reduce Readmissions for Medicare Patients With Complex Postdischarge Needs

Developed by the Care Transitions Program at the University of Colorado at Denver and AHRQ, this Solution addresses managing Medicare patients in the process of discharge to reduce readmissions. By implementing the Care Transitions Intervention program, a coach helps patients in transition from hospital to home to take an active role in their post-discharge care.
