Patient Flow Toolkit: Improving Access and Patient Flow

A toolkit developed by the Ministry of Health and Long Term Care (MOHLTC) Ontario Canada. The Emergency Department Process Improvement Program (ED PIP) is a component of Ontario's Wait Time Strategy designed to support improvements to hospital flow and build capabilities within hospitals for long term sustainable change. This toolkit provides hospitals a how-to-guide for process improvement projects and a structured approach to improve patient flow from the point patients arrive in the ED through to discharge from inpatient units. This toolkit is divided into five sections, and each section is subdivided into four subsections (Work-plan, Tools, Guidelines, and Checklist). Included in this toolkit are the Preparation Checklist, Diagnostic Checklist, Solution Design Checklist, Pilot Checklist, and Roll-out Checklist. Also, Charter Template, Work-Plan Template, Stakeholder Grid, Data Access Reporting Tool, Fishbone Diagram, Template to Document Standard Operating Procedures, and Plan-Do-Study-Act Tool Planning Form.
