The mission of North Carolina’s RACE system is to regionalize cardiac care for heart attack patients with ST-elevation myocardial infarction (STEMI) throughout the state’s acute care facilities. RACE, an acronym for Reperfusion of Acute Myocardial Infarction in Carolina Emergency Departments, aims to standardize the triage and transfer of STEMI patients much like emergency medical systems across the country handle the triage and transfer of trauma patients. RACE has set out to increase the frequency and speed of coronary reperfusion for heart attack patients by coordinating cardiac care between multiple stakeholders. The goal of RACE is to manage the rapid transfer of STEMI patients from the emergency departments (ED) of non-PCI hospitals to those facilities with catheterization labs. RACE has also helped set up protocols for non-PCI hospitals when transfers are not possible. The report is available as a PDF, requiring Adobe Reader.

Define, Design, Implement Evidence-Based Practices

Banner Health developed a Define-Design-Implement process to translate evidence into practice. Banner's process is focused on "achieving a shared baseline" in clinical processes. Two workbooks are available: 1) Implementation Guidance Workbook and 2) Clinical Practice Design Workbook. Both workbooks are available as PDFs and require Adobe Reader to view the documents.

National Center for Patient Safety 2004 Falls Toolkit

The VA National Center for Patient Safety (NCPS) developed the NCPS Falls Toolkit with the Patient Safety Center of Inquiry in Tampa, Florida, and others. The toolkit helps facilities develop a comprehensive falls prevention program. The toolkit resources are available as a PDFs, PowerPoint files, and Word files, requiring Adobe Reader, Microsoft PowerPoint, and Microsoft Word to view the files.

Registries for Evaluating Patient Outcomes: A User’s Guide: 2nd Edition

This AHRQ guide is used to aid in the establishment, management, and analyzation of patient registries in order to evaluate the real-life impact of treatments and evaluate outcomes. Users are walked through each step of the the patient registry planning and creation process. The guide also provides analysis and interpretation of the registry data. The mission of EM Xpress is to eliminate ambulance gridlock at hospitals by speeding patient offloads. Borrowing from a car racing model, EM Xpress uses “pit crews” to expedite the transfer of patient care between emergency medical services (EMS) and emergency department (ED) staff. The goal of EM Xpress is to offload patients quickly and safely so that ambulance crews can avoid delays at the ED and get back on the road to respond to other calls.
