National Center for Patient Safety 2004 Falls Toolkit

The mission of North Carolina’s RACE system is to regionalize cardiac care for heart attack patients with ST-elevation myocardial infarction (STEMI) throughout the state’s acute care facilities. RACE, an acronym for Reperfusion of Acute Myocardial Infarction in Carolina Emergency Departments, aims to standardize the triage and transfer of STEMI patients much like emergency medical systems across the country handle the triage and transfer of trauma patients.

The AHRQ along with the Kansas Foundation for Medical Care has provided an excellent quality improvement strategies and tools to assist those caring for patients dealing with cardiac and pulmonary disorders. Along with guidelines, the tools offered also include flowcharts and checklists in which will give nursing staff a better understanding, step by step on giving excellent patient care. One Size Does Not Fit All: Meeting the Health Care Needs of Diverse Populations

This report for hospitals and health care organizations provides a guide for improving the quality of care for the diverse patient groups. The report includes a self-assessment tool for organizations to assess their current care practices and services, and identify areas for improvement. The tool measures practices over four key themes: building a foundation, collecting and using data to improve services, accommodating the needs of specific populations, and establishing internal and external collaborations. RWJF developed "Expecting Success: Excellence in Cardiac Care" to focus on poor quality and inequality in care provided in U.S. hospitals and to assist and provide guides for hospitals to grade the quality of cardiac treatment they provide to patients.
