Brookings-Dartmouth Accountable Care Organization Toolkit

The purpose is to help uninsured patients with chronic medical conditions and mental health problems learn to manage their own care while finding a medical home beyond the Emergency Department. Through this program, patients work with caseworkers to find out about community providers, insurance coverage options, wellness programs and other resources in the area.

This toolkit for health care professionals helps develop a pediatric medical home and improve patient care. The elements in the toolkit help to increase outcomes, such as health status, timeliness of care, family centeredness, and family functioning in a medical home. Achieving the standards in the toolkit is a requirement to be in the National Committee for Quality Assurance Physician Practice Connections® Patient Centered Medical Home Recognition program. The toolkit was developed by the American Academy of Pediatrics and requires registration. The web-based toolkit requires a web browser.

Capacity Optimizer Tool

GE Healthcare created The Capacity Optimizer Self-Assessment Toolkit to assist with effective hospital capacity management. There are many variables to consider, which include: the number of staffed beds (medical, surgical, ICU), annual discharges, and average length of stay (ALOS). Managing these variables; such as reducing staffed beds in certain units, or increasing bed turnover, may help you achieve target utilization. The Capacity Optimizer Self-Assessment Toolkit provides data that can help hospitals develop strategies for improving utilization.

Cape Canaveral Hospital: Nursing Process Council

The Nursing Process Council developed this toolkit to eliminate barriers between staff in order to focus on quality patient care. The toolkit contains methods to identify and resolve issues related to patients within the acute care setting. The Nursing Process Council is focused on empowering staff to make the necessary decisions regarding work setting issues, that could negatively impact patient care. This toolkit provides guidelines for staff to use in order to create positive changes and redesign necessary work setting processes.

Care About Your Care: Tips for Patients When They Leave the Hospital

Developed by The Dartmouth Institute for Health Policy and Clinical Practice, along with the Robert Wood Johnson Foundation, this article explores the safe hospital to home transition for patients. The article explains the importance of coordinated care from the patient, caregiver(s), and hospital staff on the day of discharge, and how this can lead to reduced readmissions. The article also helps readers evaluate their present quality of care provided for soon-to-be discharge patients, and how to improve upon this care.

Care Coordination Forms

The New York Care Coordination Program is a collaborative partnership among State and county governments, peers and family members, and mental health provider agencies. The program offers person-centered, recovery-focused care coordination for individuals with severe mental illness (e.g., schizophrenia, bipolar disorder, clinical depression) who have had at least one psychiatric hospitalization. This toolkit was developed for the purpose of identifying, assessing, and resolving medication problems in home care for older adults. The objective is to detect potential medication problems with drug use and the symptoms associated with specific adverse drug effects. This toolkit provides the tools needed to develop an intervention plan and enable older adults to continue living safely at home. The toolkit is available as a group of web pages and PDFs and requires a web browser and Adobe Reader to view them.
