Leadership Development Toolkit

Developed by Johns Hopkins Medicine, the Leadership Development Toolkit helps structure development opportunities used to advance leadership skills of members within healthcare organizations. The Toolkit includes activities/tips, courses and references which are specific to leadership competencies measured by the Johns Hopkins Hospital Management Job Description. The toolkit is available as a group of hyperlinks. A web browser is required to view the toolkit components.

New Management and Planning Tools

A team from the Union of Japanese Scientists and Engineers (JUSE) researched and developed seven quality control tools, often called the seven management and planning (MP) tools. The ASQ has provided these MP tools to implement new creative communication and planning and managing techniques that can be used in all occupational levels. These tools will assist in identifying problems and bringing about innovative solutions. The seven MP tools are available as separate webpages and will require a web browser to view them.

Testing the Re-Engineered Hospital Discharge

Developed by Project RED (Re-Engineered Discharge), a research group at Boston University Medical Center, this project re-engineered the process of discharging patients from a hospital back into the community to make the process safer. The discharge workflow was redesigned using a set of ten components that aim to reduce post-discharge adverse events and subsequent re-hospitalizations. Care Management Unit Improves ED Flow

The Grady Health System implemented the CMU process to improve flow in the Emergency Department. This toolkit provides specific solutions to ED overcrowding and low patient satisfaction levels by focusing on patients with specific diagnosis to improve access to primary care, decrease relapse rates, decrease short stay admissions, and decrease cost. Implementing a Five-Level Emergency Severity Index Triage

The team at Hahnemann University Hospital implemented a plan where they could transition the current triage process from four to five levels. This toolkit provides critical resources by identifying patient flow quality improvement through 55 strategies and 95 associated tools. The strategies are organized into five categories: Input, Throughput, Output, Communications/Information Technology (IT), and Scheduling/Staffing. The five level ESI system provides a much more accurate description of the emergency department's patient mix.

Registries for Evaluating Patient Outcomes: A User’s Guide: 2nd Edition

This AHRQ guide is used to aid in the establishment, management, and analyzation of patient registries in order to evaluate the real-life impact of treatments and evaluate outcomes. Users are walked through each step of the the patient registry planning and creation process. The guide also provides analysis and interpretation of the registry data. The guide is available as a PDF, and requires Adobe Reader for viewing.

Reduce Waste, Fraud, and Abuse in Health Care: Fact Sheet

Developed by the AARP, this fact sheet describes the problems and highlights some of the solutions to reduce waste, fraud, and abuse in health care. The tool is available as a PDF, requiring Adobe Reader.

Integrating Chronic Care and Business Strategies in the Safety Net: A Toolkit for Primary Care Practices and Clinics

America’s safety net providers have led the way to improving the care for the chronically ill - one of the most pressing health needs of our time.To help more safety net organizations implement the Chronic Care Model effectively and sustainably, the Agency for Healthcare Research and Quality (AHRQ) contracted with Group Health's MacColl Institute, RAND Health, and the California Health Care Safety Net Institute (SNI) to develop a toolkit.

Monitoring and Evaluating Medicaid Fee-for-Service Care Management Programs: User's Guide

This User's Guide is intended to help policymakers and others understand the evidence, methodology, and issues related to evaluating the costs, quality and impact of care management programs. Readiness Assessment Tool for Nursing Homes

The Readiness Assessment Tool is designed to help nursing homes assess whether they are ready to undertake organization-wide practice and culture changes for Quality Improvement (QI). The information obtained through the baseline assessment from use of the tool can form the basis of dialogue and problem-solving across all levels of staff to enhance an organization's readiness for QI activity. It can also be used as an internal QI tool to assess the organization's progress in developing a QI culture.

Care Coordination Forms

The New York Care Coordination Program is a collaborative partnership among State and county governments, peers and family members, and mental health provider agencies. The program offers person-centered, recovery-focused care coordination for individuals with severe mental illness (e.g., schizophrenia, bipolar disorder, clinical depression) who have had at least one psychiatric hospitalization. Developed by the Canadian Collaborative Mental Health Initiative, this toolkit is a guide for health care providers wishing to establish or enhance the mental health services they provide through collaboration. Collaborative mental health care describes models of practice in which consumers, their families and caregivers, and providers from a variety of primary health care and mental health settings work together to provide better coordinated and more effective services for individuals with mental health needs.
