PRHI Readmission Reduction Guide: A Manual for Preventing Hospitalizations

Project BOOST (Better Outcomes for Older adults through Safe Transition)

Project BOOST, created in 2008 by the Society of Hospital Medicine with support from the John A. Hartford Foundation, implemented an approach to improving the hospital discharge process with the goal of decreasing readmission rates among elderly patients. This web-based toolkit will walk one through each step of designing, implementing and evaluating an intervention.

PROMETHEUS PAYMENT® Pilot Assessment and Implementation Toolkit

The Health Care Incentives Improvement Institute designed this toolkit in order to provide the necessary information to obtain a solid understanding of the PROMETHEUS Payment ® model and consider the potential benefits of implementation. This toolkit explains the beginnings of this new payment approach and provides the steps needed for implementation. Descriptions are given on how to conduct your initial data analysis, build support among leaders, engage your providers and payers, define your project scope and launch your pilot. Protect Your Patients. Protect Yourself Toolkit

This toolkit features Best Practice models and multiple resource materials, that infection control specialists can use to encourage health care institutions to implement or expand their health care worker immunization programs. Public Health Emergency Exercise Toolkit

Emergency preparedness requires careful planning, including clearly identifying long-term improvement goals and specific exercise objectives and then designing, developing, conducting, and evaluating each exercise accordingly. This toolkit provides templates, checklists, and forms to assist with every step of the exercise process to test the implementation of emergency management procedures and protocols, fine-tune the internal coordination of the emergency plan, and practice coordinating with external response sectors. RACE System

The mission of North Carolina’s RACE system is to regionalize cardiac care for heart attack patients with ST-elevation myocardial infarction (STEMI) throughout the state’s acute care facilities. RACE, an acronym for Reperfusion of Acute Myocardial Infarction in Carolina Emergency Departments, aims to standardize the triage and transfer of STEMI patients much like emergency medical systems across the country handle the triage and transfer of trauma patients.

Re-Engineered Discharge (RED) Toolkit - Updated Version

Researchers at the Boston University Medical Center (BUMC) developed the Re-Engineered Discharge (RED) and it was effective at reducing readmissions and emergency department (ED) visits. The Agency for Healthcare Research and Quality (AHRQ) contracted with BUMC to develop this new toolkit to assist hospitals, particularly those that serve diverse populations, to replicate the RED. This toolkit is an enhancement of previous RED resources and introduces a new RED component on overcoming language barriers.

Readiness Assessment Tool for Nursing Homes

The Readiness Assessment Tool is designed to help nursing homes assess whether they are ready to undertake organization-wide practice and culture changes for Quality Improvement (QI). The information obtained through the baseline assessment from use of the tool can form the basis of dialogue and problem-solving across all levels of staff to enhance an organization's readiness for QI activity. It can also be used as an internal QI tool to assess the organization's progress in developing a QI culture.
