The Fall T.I.P.S. Toolkit (FTTK) is a web-based program for integrating documentation and communication of fall risk assessment. The purpose of the toolkit is to help nurses and others communicate fall risk status and planned interventions and to make alerts accessible to all care team members. The toolkit was developed by The Center for Nursing Excellence at the Brigham and Women's Hospital. The toolkit is available as several PDFs, requiring Adobe Reader to view the document.
