IHI Global Trigger Tool for Measuring Adverse Events (Second Edition)

The IHI Global Trigger Tool for Measuring Adverse Events provides an easy-to-use method for accurately identifying adverse events (harm) and measuring the rate of adverse events over time. Tracking adverse events over time is a useful way to tell if changes being made are improving the safety of the care processes.

The Trigger Tool methodology includes a retrospective review of a random sample of patient records using “triggers” (or clues) to identify possible adverse events. Many hospitals have used this tool to identify adverse events, to measure the level of harm from each adverse event, and to identify areas for improvement in their organizations. It is important to note, however, that the IHI Global Trigger Tool is not meant to identify every single adverse event in a patient record.

The recommended time limitation for review and the random selection of records are designed to produce a sampling approach that is sufficient for the design of safety work in the hospital.

	Seven Steps to Patient Safety Healthcare organizations operate within a complex healthcare system which can brings risks and evidence shows that things will and do go wrong. The National Patient Safety Agency created this toolkit to address these risks and make improvements in distinct areas of medical practice in order to meet clinical governance, risk management and controls assurance targets. This tool includes a list of potential AE triggers and instructions for collecting needed information to measure the rate of AEs in a NICU. For example - the total number of AEs per 100 admissions - and the percentage of admissions with an AE.
	Cost of MRSA Events Tool GE Healthcare created this calculator tool to assist in the reduction of hospital acquired infection (HAI). Reducing HAI will result in safer, higher quality care for each patient and fewer infections also mean reduced costs. Methicillin-Resistant Staphylococcus Aureus (MRSA) is a bacterial infection that can be reduced greatly through improved hand hygiene compliance. Each MRSA infection results in extra pain and care for the patient and the infections can become an extra cost for the hospital to absorb.
