NTOCC has created information to help healthcare professionals understand how poor transitions impact care delivery and how you can help improve transitions in your facility. NTOCC developed an implementation and evaluation outline for transitions of care tools and resources for institutions, to raise awareness about transitions of care among health care professionals, government leaders, and consumers to increase the quality of care, reduce medication errors, and enhance clinical outcomes. It provides an overall healthy work environment score and scores for each of the six essential standards. This screening tool evaluates a work environment based on each of the standards, but does not diagnose specific challenges within a standard.
	Toolkit for ICUs to Improve the Safety and Quality of Patient Care The toolkit was developed by an Intensive Care Unit Implementation Workgroup (including including physicians, nurses, allied health professionals and hospital administrators) and supported by a team of Michigan Health and Safety Coalition (MH&SC) members, technical and policy experts and administrative staff. This web-based toolkit helps hospitals achieve ICU physician staffing standards.
