Keeping Kidney Patients Safe: Patient Safety Improvement Toolkit

The free toolkit includes planning tools, information and promotional materials to assist in planning and implementing a safety improvement program.
This toolkit also assists your hospital in fulfilling the CMS Conditions for Coverage for End Stage Renal Disease (ESRD) Facilities.
In the spring of 2006, the Kidney & Urology Foundation of America, Inc., and the Renal Physicians Association and in cooperation with the Centers for Medicare & Medicaid Services co-sponsored a survey of nephrology professionals and kidney patients to collect information about patient safety. Through the survey, six primary areas of needed improvement were identified. With this information, the ESRD Patient Safety Initiative developed KidneyPatientSafety.org to provide renal healthcare professionals with tools to identify possible problem areas, train staff and implement changes to improve patient safety and reduce errors.
The Keeping Kidney Patients Safe website is a resource for nephrology professionals who are dedicated to kidney patient safety. The free toolkit includes planning tools, information and promotional materials to assist in planning and implementing a safety improvement program to help fulfill the CMS Conditions for Coverage for End Stage Renal Disease (ESRD) Facilities.
