The free toolkit includes planning tools, information and promotional materials to assist in planning and implementing a safety improvement program.

This toolkit also assists your hospital in fulfilling the CMS Conditions for Coverage for End Stage Renal Disease (ESRD) Facilities.

In the spring of 2006, the Kidney & Urology Foundation of America, Inc., and the Renal Physicians Association and in cooperation with the Centers for Medicare & Medicaid Services co-sponsored a survey of nephrology professionals and kidney patients to collect information about patient safety. Through the survey, six primary areas of needed improvement were identified. With this information, the ESRD Patient Safety Initiative developed KidneyPatientSafety.org to provide renal healthcare professionals with tools to identify possible problem areas, train staff and implement changes to improve patient safety and reduce errors.

The Keeping Kidney Patients Safe website is a resource for nephrology professionals who are dedicated to kidney patient safety. The free toolkit includes planning tools, information and promotional materials to assist in planning and implementing a safety improvement program to help fulfill the CMS Conditions for Coverage for End Stage Renal Disease (ESRD) Facilities.

AACN Healthy Work Environment Assessment

Developed by the American Association of Critical-Care Nurses, the AACN Healthy Work Environment Assessment tool is a screening test for organizations to measure the health of their work environment against the AACN Standards for Establishing and Sustaining Healthy Work Environments. It provides an overall healthy work environment score and scores for each of the six essential standards. This screening tool evaluates a work environment based on each of the standards, but does not diagnose specific challenges within a standard.

Toolkit for ICUs to Improve the Safety and Quality of Patient Care

The toolkit was developed by an Intensive Care Unit Implementation Workgroup (including including physicians, nurses, allied health professionals and hospital administrators) and supported by a team of Michigan Health and Safety Coalition (MH&SC) members, technical and policy experts and administrative staff. This web-based toolkit helps hospitals achieve ICU physician staffing standards. AACN Standards for Establishing and Sustaining Healthy Work Environments

This document is the executive summary of the American Association of Critical-Care Nurses (AACN) Standards for Establishing and Sustaining Healthy Work Environments. This tool presents six standards required to establish and sustain a healthy work environment. The standards are evidenced-based and highlight principles of professional performance.
