SCOAP Ambulatory Checklist

The Surgical Care and Outcomes Assessment Program (SCOAP) Surgical Checklist Initiative was created by a coalition of major health care stakeholders in Washington State. Through this initiative, they developed the SCOAP Surgical Checklist and the SCOAP Ambulatory Checklist. These checklists are an active way of controlling the variability that exists in surgical care, thus creating a system that delivers safer surgery. 	AACN Healthy Work Environment Assessment Developed by the American Association of Critical-Care Nurses, the AACN Healthy Work Environment Assessment tool is a screening test for organizations to measure the health of their work environment against the AACN Standards for Establishing and Sustaining Healthy Work Environments. It provides an overall healthy work environment score and scores for each of the six essential standards. This screening tool evaluates a work environment based on each of the standards, but does not diagnose specific challenges within a standard.
	Toolkit for ICUs to Improve the Safety and Quality of Patient Care The toolkit was developed by an Intensive Care Unit Implementation Workgroup (including including physicians, nurses, allied health professionals and hospital administrators) and supported by a team of Michigan Health and Safety Coalition (MH&SC) members, technical and policy experts and administrative staff. This web-based toolkit helps hospitals achieve ICU physician staffing standards. The toolkit helps hospitals improve ICU structures and processes by adopting evidence-based best practices which meet the MH&SC ICU Physician Staffing Guideline.

	AACN Standards for Establishing and Sustaining Healthy Work Environments This document is the executive summary of the American Association of Critical-Care Nurses (AACN) Standards for Establishing and Sustaining Healthy Work Environments. This tool presents six standards required to establish and sustain a healthy work environment. The standards are evidenced-based and highlight principles of professional performance. 	Patient Safety Program - Debriefs Toolkit Developed by the U.S. Department of Defense Military Health System, the Debriefs Toolkit provides guidelines for facilitating retrospective team events to share view of performance and promote opportunities for continuous learning and process improvement. The toolkit is available as several PDFs, Word files, and Windows Media Videos (WMVs), requiring Adobe Reader, Microsoft Word, and Windows Media Player, respectively.
	Patient Safety Program - Patient Falls Reduction Toolkit Developed by the U.S. Department of Defense Military Health System, the Patient Falls Reduction Toolkit addresses four aspects of a Patient Falls Reduction Program; assessment and reassessment of patients at risk for falls, interventions to prevent falls, patient and family education, and data collection supporting continuous improvement.
