Drinking and Reproductive Health

This is an intervention toolkit developed to help women’s health care clinicians prevent Fetal Alcohol Spectrum Disorders ( FASD). It is not a clinical diagnostic tool, but includes a spectrum of conditions that may occur as a result of prenatal alcohol exposure. This toolkit provides the clinician with strategies to reduce alcohol exposure to the dev eloping fetus. It consists of screening, education and counseling information for pregnant women that can be incorporated into routine care.

Safe Injection Practices Toolkit

The ASC Quality Collaboration has created this toolkit to improve the current administration of injections. Safe injection practices are critical to patient safety and also protect healthcare providers. There is a risk that Hepatitis C and hepatitis B viruses, as well as HIV can be spread from patient to patient without improvements in the injection practices. There are two toolkits – the “BASIC safe Injection Practices” and the “EXPANDED Safe Injection Practices." This toolkit includes various resources to help practices become more effective in recruiting ophthalmic medical personnel, as well as guides for retaining them.

MARR Long-Term Care Tool Kit

The MARR Coalition designed this toolkit to help health care providers caring for patients in long-term care facilities. The toolkit is based on the Centers for Disease Control and Prevention’s -12 Steps to Prevent Antimicrobial Resistance Among Long-Term Care Residents, as well as their recommendations. This toolkit includes: Guides on how to break specific links in the chain of infection, practical information, protocols, policies, and tools which are designed to be easily modified for individual facility needs.

ASHP Medication Reconciliation (Med Rec) Toolkit

The ASHP developed the “ASHP Medication Reconciliation (Med Rec) Toolkit” to provide educational materials for its members in order to improve the transitions of care, recognized as critical steps in the management of patients. This toolkit will provide ASHP members with tools, references, and recommendations as well as ideas and examples of success stories and lessons learned to help with providing quality care for their patients and meeting the National Patient Safety Goal. This toolkit was created by the AHIMA in order to guide healthcare organizations, HIM professionals and (IT) professionals with handling copy technology in the EHR environment. This toolkit is a critical aid for developing policies, procedures and sound documentation integrity policies within their organization. Pediatric Intensive Care Unit (PICU) Trigger Tool
