	Child & Adolescent Obesity Provider Toolkit The California Medical Association (CMA) Foundation and California Association of Health Plans (CAHP) developed this toolkit to assist in managing Obesity in children and adolescents. These guidelines will assist health care providers to address the prevention, early identification, weight management education of overweight and obese children and adolescents. 	LEAN Works! Leading Employees to Activity and Nutrition This resource offers interactive tools and evidence-based strategies to design effective worksite obesity prevention and control programs, including an obesity cost calculator that companies can use to estimate how much obesity is costing them and how much savings they could reap with different workplace interventions. This tool is offered by the CDC and requires a web browser for viewing.
	Childhood Obesity Tools This web-based toolkit was developed by the National Initiative for Children’s Healthcare Quality and the teams from NICHQ’s Learning Collaborative Healthy Care for Healthy Kids; in order to provide guides to assist primary care physicians deliver organized, integrated and specialized services to prevent obesity and improve care for children who are already overweight or at risk for overweight. This toolkit is available as several PDFs, requiring Adobe Reader to view the documents.
	Pediatric Obesity and Diabetes Prevention Kit Developed by Independence Blue Cross, in collaboration with the American Academy of Pediatrics and the American Diabetes Association, this toolkit is designed for practitioners treating pediatric patients from ages 2 to 18. The contents of this toolkit are available in both English and Spanish. The purpose of this toolkit is to provide practitioners with materials that will aid in confronting the pediatric obesity and diabetic epidemic.
