Healthcare organizations operate within a complex healthcare system which can brings risks and evidence shows that things will and do go wrong. The National Patient Safety Agency created this toolkit to address these risks and make improvements in distinct areas of medical practice in order to meet clinical governance, risk management and controls assurance targets. This online toolkit offers various exercises to develop safety strategies and tips for safe care in the general practice. There are four guides in the series that provide checklists to help staff plan their activities and measure patient safety performance. This toolkit consists of webinars and PDFs, requiring a web browser and Adobe Reader.

Keeping Kidney Patients Safe: Patient Safety Improvement Toolkit

University of Michigan Health System Patient Safety Toolkit

This web-based toolkit is designed to build a foundation of knowledge and to suggest practical applications for developing best practices. Content includes conceptual discussions, seminal literature, and tools and templates, integrating original material and material adapted from other sources. The toolkit contains chapters related to specific topics that are considered to be of prime importance to hospitals seeking to enhance patient safety and quality care. The toolkit was developed by the University of Michigan Health System. The toolkit requires a web browser.

Medication Safety Reconciliation Toolkit

The Medication Reconciliation Toolkit helps hospitals establish and implement a standardized medication reconciliation process. The toolkit provides guidance, sample forms, and tips. The toolkit is available as a PDF, requiring Adobe Reader.

MATCH - Medication Reconciliation Toolkit

Northwestern Memorial Hospital developed a web-based, comprehensive, multidisciplinary team approach to evaluate the extent of adverse events and develop interventions to improve patient safety and quality of care.

A MATCH team includes physicians, nurses, pharmacists, health services researchers and experts in quality and patient safety. This toolkit is a culmination of Northwestern Memorial Hospital's experiences as well as collaborations with other organizations and individuals locally, regionally and nationally.

Medication Reconciliation Essential Data Specifications

This resource can help health care providers collect, communicate, and manage critical information needed for medication reconciliation when patients move from one practice setting or level of care to another. Data collection may be electronic or on paper. This information can be used to reduce medication errors and ensure safe transitions. The tool is available as a PDF, requiring Adobe Reader.

SCOAP Ambulatory Checklist

The Surgical Care and Outcomes Assessment Program (SCOAP) Surgical Checklist Initiative was created by a coalition of major health care stakeholders in Washington State. Through this initiative, they developed the SCOAP Surgical Checklist and the SCOAP Ambulatory Checklist. These checklists are an active way of controlling the variability that exists in surgical care, thus creating a system that delivers safer surgery. WHO Surgical Safety Checklist and Implementation Manual 2009 Edition

The World Health Organization (WHO) Surgical Safety Checklist reinforces accepted safety practices and fosters better communication and teamwork between clinical disciplines. The checklist is not a regulatory device or a component of official policy; it is intended as a tool for use by clinicians interested in improving the safety of their operations and reducing unnecessary surgical deaths and complications. An IHI developed Trigger tool aimed at closing the gap between safety and quality, and the physical environment in which care is provided. Survey User’s Guide - Nursing Home Survey on Patient Safety Culture

A group from Westat created this survey as a general overview of the issues and major decisions that are involved in conducting a survey – collecting data procedures and then preparing a report of the results. Part One contains a Survey User’s Guide and Part Two has a list of User’s Tools. This toolkit is designed to provide nursing homes with the basic knowledge and tools needed to conduct a safety culture assessment, along with ideas for using the data.

Improving on Transitions of Care: How to Implement and Evaluate a Plan

NTOCC has created information to help healthcare professionals understand how poor transitions impact care delivery and how you can help improve transitions in your facility. NTOCC developed an implementation and evaluation outline for transitions of care tools and resources for institutions, to raise awareness about transitions of care among health care professionals, government leaders, and consumers to increase the quality of care, reduce medication errors, and enhance clinical outcomes.
