Developed by The Renal Network, the Fistula Improvement Quality Improvement Project (QIP) consists of two documents; RCA 5Whys Tool Form and Fistula Improvement Action Plan. These forms cover how to assess your current situation and analyzing the source of any problems by "asking why?" Once the source of the problem is identified and addressed appropriately, low AV fistula rates should also disappear. The Action Plan form helps in managing the quality improvement process. Developed by The Renal Network, the Catheter Reduction Quality Improvement Project (QIP) consists of two documents; Catheter Reduction 5Whys Tool Form and Catheter Reduction Action Plan. Pyromaker: Online Application to Identify Gene Mutations

Johns Hopkins researchers have developed an easy-to-use online computer software application that can identify meaningful gene mutations faster and at less cost than other methods currently available. The software generates simulated pyrograms, which are readouts from a gene sequencing technique known as pyrosequencing. “Pyromaker,” is available free-of-charge.
