The Health Information Technology (HIT) Toolkit for Critical Access and Small Hospitals helps these health care organizations assess their readiness and plan, select, implement, make effective use of, and exchange important information about the clients they serve. The toolkit contains numerous resources, including tools for telehealth, health information exchange, and personal health records. The toolkit was developed by Stratis Health. The toolkit includes web-based components, Word documents, and PowerPoint documents, requiring a web browser, Microsoft Word, and Microsoft PowerPoint.

Improving on Transitions of Care: How to Implement and Evaluate a Plan

NTOCC has created information to help healthcare professionals understand how poor transitions impact care delivery and how you can help improve transitions in your facility. NTOCC developed an implementation and evaluation outline for transitions of care tools and resources for institutions, to raise awareness about transitions of care among health care professionals, government leaders, and consumers to increase the quality of care, reduce medication errors, and enhance clinical outcomes.

Patient Safety Toolbox

The NASHP, supported by the Commonwealth Fund, developed this online toolkit to be used or modified to help organizations develop or improve adverse event reporting systems.  The toolkit includes: Background Information, Data Collection Tools, Data Analysis Tools, Data Feedback Tools, NASHP Publications on Patient Safety and a State Map.
